Welcome to our online Psychiatric Clinic for Children & Adults (Ages 2-99), for medication management.

Our Psychiatric Mental Health Nurse Practitioners diagnose and treat anxiety, ADHD, depression, bipolar, mood disorders, schizophrenia, addiction, sleep disorders, migraines and more across Arizona and Wisconsin.

In addition to being a clinic, Alay is also a teaching facility. In addition to our providers, advanced practice registered nurses work as part of our team. APRNs pursuing certification as Psychiatric Mental Health Nurse Practitioners and as Board Certified Psychiatric Mental Health Nurse Practitioners have the opportunity to be taught and encouraged by our providers/preceptors. 

This is an Outpatient Clinic, in order to see your provider you must make an appointment. Our clinic is not designed or equipped for emergency response. If you need immediate assistance with an urgent medical or mental health issue, dial 911 or go to your nearest emergency room.

Getting Started

Step 1: Get The Information You Need

It’s normal to have a lot of questions when researching treatment options. To get started, all new patients begin with an Intake appointment with a Psychiatric Mental Health Nurse Practitioner. This is a 60 minute telehealth appointment where we gather detailed information about your specific symptoms, previous medications taken, and background history. Appointments following your initial appointment are 15-30 minutes long and you are rescheduled based on what you and your provider agree upon. By the end of the session, you and your provider can start developing a personalized treatment plan. Or, if Alay Health Team is not the right fit, we’ll do our best to suggest other options for you.

Step 2: Fill out New Patient Forms via our Intake Portal

Requesting your first appointment is simple. Go to our New Patient intake portal. Once we receive your information – including a photo of your insurance card, a staff member of our team will give you a call to get you set up as a new patient. Your intake appointment will then be confirmed, and you’ll receive a a link to create an account with our patient portal.

Step 3: Complete New Patient Paperwork

Prior to your first appointment, you’ll be sent a link to create a password with our Patient Portal (the patients name and date of birth are required to complete this part, the email address you received the link to is your username) to review our consent forms and office policies. If your forms need to be printed, please go to our telehealth page to download those and send them back to us. If your forms are not completed prior to 24 hours before you initial appointment, that appointment will be cancelled.

Step 4: Attend Your Appointment

You will receive an email, phone call and text message reminding you of your upcoming appointment. 24 hours before your appointment, you will receive an email with a unique link to your video session; it look like just a reminder email, but your link is also in there. Our system automatically sends the links out, and staff can view at what time it was sent out.
Approx 10 minutes before your start time, just click the link or copy-paste it into your internet browser. Follow the instructions on the screen to enter the video session. Your clinician will join you there at the scheduled time. If you need more help, please visit our Help page. Please remember that sometimes providers will run behind, as providers do in an actual office, please give the provider 5-10 minutes to join before you call the office, and if the provider does not join by then, please call the office immediately so that staff can investigate, if you call an hour, or a day later, it is more difficult to find out what occurred.
